Responding to an Audit Questionnaire

How to find an audit questionnaire, answer its questions, attach evidence, submit a universe and request more time.

When a health plan audits your organization, the request arrives in Readily as a questionnaire. This guide covers responding to one: finding it, answering the questions, attaching evidence, submitting a universe, and asking for more time.

The numbered markers in each screenshot match the numbered steps beneath it.

1. Find Your Questionnaires

You will get an email when a request is issued. It links straight into Readily, or you can go to Audit Questionnaires in the sidebar at any time.

Steps:

  1. Check Status to see where each request stands, from Pending through Submitted.

  2. Check Due Date for the deadline on the most recent request.

  3. Check Progress to see how much of the questionnaire is answered.

  4. Click Acknowledge all to confirm receipt of anything still waiting on it.

Please note: Acknowledging a request only confirms you have received it. It does not answer anything, and the due date still applies.

2. Open a Questionnaire

Opening a request shows what you owe in total at the top, then each collection that makes it up. A collection groups one kind of request — policy and procedure questions, a case file request, or a universe request — and each has its own due date, its own progress and its own Submit.

Steps:

  1. Documents & Analysis lists everything you have uploaded.

  2. Comments & Extensions is where you ask the auditor a question or request more time.

  3. Export to Excel takes the questionnaire offline so you can circulate it internally.

  4. Invite Team Members brings colleagues in to help.

  5. View Report opens an assessment the auditor has published to you.

Each collection shows how many answers are Submitted against how many still Need Response, so you can see at a glance what is left.

3. Answer the Questions

Expand a collection to see its questions, grouped by element. Each row carries the requirement, the evidence attached so far, the auditor's assessment once they have reviewed it, and who on your team it is assigned to.

Steps:

  1. Drag your policy documents onto Upload & analyze. Readily reads them, drafts an answer for every question and cites the passage it came from, so you are editing a draft rather than starting from nothing.

  2. Use Bulk Assign to hand a set of questions to colleagues.

  3. Use Bulk Upload Evidence to attach files across several questions at once.

Best practice: Upload your documents first and let the analysis draft the answers, then review each one. It is faster than answering question by question, and it attaches the evidence at the same time.

Please note: Answers save as you work, but the auditor does not see them until you press Submit on that collection. Submitting one collection does not submit the others.

4. Submit a Universe

A universe request asks for a complete list of records for the audit period — every claim, denial or credentialing file — as a spreadsheet. Open the universe collection, download the template if one is provided, fill it in and upload it.

Readily checks the file as it arrives and tells you immediately if a column is missing, a date falls outside the review period or the format does not match. Fix and re-upload; you do not have to wait for the auditor to tell you.

Please note: A universe collection can show Resubmit after you have already submitted. That means the auditor has asked for a corrected or updated file.

5. Ask a Question or Request More Time

Open Comments & Extensions. Post a comment to reach the auditor about anything in the questionnaire, or request a new due date for a collection. The auditor approves or declines, and the original due date stays on the record alongside any extension.

What Happens After You Submit

The auditor reviews what you sent and records an assessment against each requirement, which appears in the Auditor Assessment column. If something is missing they issue another request, and the collection reopens with only the outstanding items needing a response.

Published assessments appear under ASSESSMENT REPORTS at the top of the questionnaire, showing how many requirements were met and how many still need documents.
